Caregiver Specialist

We are seeking a dedicated and compassionate Direct Support Professional to join our team at Hearts & Hands of Care, Inc. As a Direct Support Professional, you will play a vital role in providing ongoing services to clients and individuals with needs in their community or in their home.
Key Responsibilities- Provide high-quality care and support to clients, promoting their independence and well-being.
- Develop and implement individualized service plans, working collaboratively with clients, families, and team members.
- Document daily activities, progress, and challenges, ensuring accurate and comprehensive records.
- Engage in friendly, professional relationships with clients, families, and colleagues, maintaining open communication and boundaries.
- Stay up-to-date with knowledge of community resources, services, and supports, ensuring clients receive the best possible care.
- Coordinate transportation for clients, ensuring safe and reliable transportation.
- Interact professionally with stakeholders, including families, healthcare providers, and community agencies.
- Work collaboratively with the Child and Family Team to promote community-based care and support the most appropriate and least restrictive settings.
- Assist clients in transitioning through life domains, services, levels of care, and into adulthood.
- Demonstrate excellent written documentation, using correct spelling, grammar, and therapeutic terminology.
- Participate in team work and shared leadership, communicating effectively with colleagues and supervisors.
- Must be at least 18 years of age.
- Meet all requirements for the position as set out in 7 AAC – 7 AAC.
- Pass a Criminal Background Check.
- Not have been denied a healthcare provider license or certification for a reason related to patient services.
- Be able to independently assist clients with specific ADLs.
- Attend training and obtain a First Aid card from the American Red Cross, American Heart Association, or other agency approved by The Department of Seniors and Disability Services.
- Attend Mandt training.
- Have confirmed service in the industry with advanced training in behavioral, medical, or caregiver services.
