Job Summary

We are seeking a dedicated and compassionate Direct Support Professional to join our team at Hearts & Hands of Care, Inc. As a Direct Support Professional, you will play a vital role in providing ongoing services to clients and individuals with needs in their community or in their home.

Key Responsibilities
  • Provide high-quality care and support to clients, promoting their independence and well-being.
  • Develop and implement individualized service plans, working collaboratively with clients, families, and team members.
  • Document daily activities, progress, and challenges, ensuring accurate and comprehensive records.
  • Engage in friendly, professional relationships with clients, families, and colleagues, maintaining open communication and boundaries.
  • Stay up-to-date with knowledge of community resources, services, and supports, ensuring clients receive the best possible care.
  • Coordinate transportation for clients, ensuring safe and reliable transportation.
  • Interact professionally with stakeholders, including families, healthcare providers, and community agencies.
  • Work collaboratively with the Child and Family Team to promote community-based care and support the most appropriate and least restrictive settings.
  • Assist clients in transitioning through life domains, services, levels of care, and into adulthood.
  • Demonstrate excellent written documentation, using correct spelling, grammar, and therapeutic terminology.
  • Participate in team work and shared leadership, communicating effectively with colleagues and supervisors.
Requirements
  • Must be at least 18 years of age.
  • Meet all requirements for the position as set out in 7 AAC – 7 AAC.
  • Pass a Criminal Background Check.
  • Not have been denied a healthcare provider license or certification for a reason related to patient services.
  • Be able to independently assist clients with specific ADLs.
  • Attend training and obtain a First Aid card from the American Red Cross, American Heart Association, or other agency approved by The Department of Seniors and Disability Services.
  • Attend Mandt training.
  • Have confirmed service in the industry with advanced training in behavioral, medical, or caregiver services.
  • Be reliable and dependable.
