Patient Care Coordinator

We are seeking a compassionate and highly organized Patient Care Coordinator to join our team. In this role, you will play a vital part in ensuring seamless transitions of care for patients as they move between different healthcare settings.

Responsibilities:
  • Coordinate and facilitate the smooth transfer of patients from acute or observation care to their next destination, whether it be home, rehabilitation, or another facility.
  • Conduct thorough assessments of patient needs, taking into account their functional abilities, cultural background, and available support systems.
  • Collaborate with a multidisciplinary healthcare team to develop, implement, and manage comprehensive care plans tailored to each patient's unique requirements.
  • Maintain meticulous documentation of all interventions and care plan elements within the patient's medical record, ensuring accuracy and timeliness.
  • Provide unwavering support and advocacy for patients and their families throughout their healthcare journey.
  • Serve as a liaison between patients, healthcare facilities, and community resources, connecting individuals with essential services and benefits.
  • Stay informed about Medicare, Medicaid, and other relevant programs to guide patients in making informed decisions regarding their care and discharge planning.
  • Demonstrate expertise in contemporary behavioral health principles, end-of-life dynamics, grief counseling, substance abuse identification and intervention, and support for victims of abuse, neglect, or violence.
  • Contribute to the professional development of staff by providing educational resources and guidance.
Qualifications:

A Bachelor's Degree in Social Work is required. A valid Alaska Licensed Behavioral Health Specialist (LBSW) license is mandatory within 12 months of hire. A temporary LBSW license is acceptable during the first year of employment.

Candidates must possess a comprehensive understanding of government and community agencies, resources such as Medicare/Medicaid, long-term care, and other applicable services. Exceptional communication, interpersonal, and time management skills are essential. In acute care settings, flexibility to work varying hours and participate in after-hours/weekend call rotations is required.

Preferred Qualifications:

  • Basic Life Support (BLS) Certification
  • Additional related education and/or experience
