Physical Therapist Home Health Specialist

Fairbanks Memorial Hospital is seeking a skilled Physical Therapist to join our Home Health Care team in Alaska's interior. As a Physical Therapist, you will play a vital role in helping patients regain their independence and improve their quality of life.
Responsibilities- Evaluate patients' functional abilities and limitations to determine the need for intervention.
- Develop and implement individualized treatment plans to achieve patient goals.
- Provide skilled treatment in a safe and effective manner, taking into account patients' physical, social, spiritual, cultural, educational, and age-specific needs.
- Assess ongoing effectiveness of treatment and modify plans as needed.
- Communicate with patients, families, caregivers, and interdisciplinary teams to ensure seamless care.
- Document patient progress and treatment plans according to professional practice guidelines.
- Graduation from an APTA-accredited school of Physical Therapy or equivalent.
- Current Physical Therapy license within the State of Alaska.
- BLS certification required.
- Ability to become proficient with computer software applications commonly used in an automated patient care environment.
