About the Role

We are seeking a highly motivated and compassionate Re-Entry Support Specialist to join our team at Open Sky Community Services. As a key member of our After Incarceration Support Program, you will play a vital role in supporting the re-entry of returning citizens and individuals with a history of justice system involvement.

Key Responsibilities
  • Intake and Assessment: Welcome individuals and families into our program, complete intakes and initial needs assessments, and gather health insurance information and other eligibility requirements.
  • Data Entry and Management: Assist with data entry for the program, including RedCap, EIM/ESM, ehana, and other systems.
  • Resource Coordination: Evaluate individuals' needs and service preferences, identify current resources and services, and complete all required documentation in a timely manner.
Qualifications
  • Education: High School Diploma, GED, or equivalent required.
  • Experience: Lived experience or minimum of 1 year experience working with people experiencing homelessness, justice involvement, serious and persistent mental illness, substance use disorder, or other underserved populations required.
  • Driver's License: Valid driver's license and acceptable driving record required.
