Youth Transition Support Specialist

The Youth Transition Support Specialist operates under the guidance of the TIL Program. This role is essential in engaging and retaining young individuals in aftercare, specifically those referred by LUK's placement initiatives. Through comprehensive assessments and consultations, the Youth Transition Support Specialist collaborates with each young person to formulate and execute tailored aftercare plans and objectives. Regular evaluations of progress towards these goals are integral to this position.
This role is pivotal in fostering connections between LUK and vital community partners, assisting young individuals in reaching their aspirations while upholding LUK's core values.
Responsibilities:
- Provide aftercare case management, referrals, and advocacy to promote the well-being and development of youth in alignment with LUK's policies.
- Utilize available resources within a diverse community effectively to create the most suitable service plan for each young person.
Qualifications: A Bachelor's Degree in Human Services or a related discipline is preferred. This position demands a strong commitment to the preservation and reunification of young adults, alongside a solid understanding of childhood trauma and developmental processes. A valid driver's license and a legally registered vehicle are required.
