Qualifications:

BA/BS degree in a field related to human services with a minimum of 1 year of experience or High School diploma/GED with a minimum of 3 years of experience working with adults in community-based and/or medical environments.

Must attend annual mandatory in-service and educational programs.
Spanish speaking is preferred but not required.

Proficient computer skills in contemporary Windows operating systems and Microsoft Office Suite, particularly Word and Excel; ability to learn new systems; experience with data entry and management; and comfort with mobile technologies.

Ability to openly address and acknowledge issues related to substance use and mental health.
Knowledge of psychiatric rehabilitation and/or substance use, along with an understanding of the recovery model.
Must be able to perform each essential duty satisfactorily.
Preference for candidates with a demonstrated understanding of and competence in serving culturally diverse populations.
Must possess a valid driver's license and have access to an operational and insured vehicle for transporting clients.

The Client Care Specialist is responsible for: Utilizing effective, respectful, empowering, and innovative engagement strategies to ensure clients are at the center of their Behavioral Health Care Plan (BHCP) services.

Conducting outreach and engagement activities with assigned clients and assisting them in enrolling in the BHCP program.

Completing comprehensive assessments collaboratively with engaged clients, under the supervision of the Clinical Care Manager, and with input from Care Team members and other stakeholders.

Conducting annual re-assessments.

Developing individualized treatment plans with each engaged client, under the supervision of the Clinical Care Manager, and with input from Care Team members and other stakeholders.

Updating treatment plans according to required timelines.
Creating advanced directives, acute care plans, and/or crisis plans with engaged clients as necessary.
Collaborating with engaged clients to assemble Care Teams and facilitate all communication and coordination with the team.

Supporting engaged clients during care transitions, including attendance at discharge planning meetings, face-to-face meetings post-discharge, ensuring linkages with all necessary services and supports, and facilitating client participation in those services.

Assisting the RN with medication reconciliation functions as required, such as information collection.

Providing health and wellness coaching to engaged clients and assisting them in identifying and utilizing health and wellness resources in the community.

Connecting engaged clients to all necessary services and supports, including those that address social needs affecting health, and facilitating ongoing connections.

Collaborating with existing providers, Care Team members, state agency staff, and all other stakeholders to deliver supports and activities in accordance with the client's person-centered treatment plan.
