TITLE :
COMMUNITY RELATIONS OUTREACH COORDINATOR - COMMUNITY ENGAGEMENT DEPARTMENT

POSITION SUMMARY:
Under the guidance of the PCA Community Relations Manager, the Community Relations Outreach Coordinator plays a pivotal role in enhancing access to aging services for ethnically diverse seniors and their caregivers. The primary goal is to cultivate strong partnerships between PCA and healthcare providers, community organizations, older adults, and their caregivers. This role is essential in connecting diverse communities to PCA and other aging-related programs, benefits, and entitlements. The Coordinator will facilitate the delivery of social services to these populations by offering information, referrals, advocacy, training, and technical support. The secondary objective is to provide outreach, assistance, and collaboration with leaders from various ethnically diverse communities to enhance access to resources and services for these groups.

REQUIRED QUALIFICATIONS:

Education:
  • Bachelor's Degree in a relevant field (Social Work, Gerontology, or related Social Science) or equivalent education and experience.
Experience:
  • Minimum of three years of experience in community outreach, particularly with aging and ethnically diverse populations.
  • Familiarity with aging services is preferred.
Professional Attributes:
  • Exceptional interpersonal skills with a sensitivity to individuals from various ethnic backgrounds.
  • Strong verbal communication skills, including public speaking capabilities.
  • Ability to build and maintain effective relationships with external partners, such as referral sources and community leaders.
  • Self-driven and capable of working independently with minimal oversight.
  • Strong advocacy skills, committed to enhancing consumer services.
  • Excellent analytical and problem-solving abilities, open to sharing ideas for program and service enhancements.
  • Highly organized with the capacity to manage multiple tasks efficiently.
  • Proficient administrative skills, including planning and coordinating meetings and events.
  • Strong written communication and computer proficiency.
  • Collaborative team player, working effectively within the Community Engagement department and with other PCA divisions.
  • Flexibility to work a varied schedule to accommodate outreach opportunities, often outside regular hours.
  • Bilingual in English and Spanish.

Additional Requirements:
  • Must hold and maintain a valid driver's license.


KEY RESPONSIBILITIES:


a. Receive referrals from community organizations and leaders, acting as a liaison between PCA and other service providers.

b. Educate target populations about PCA and the aging network's offerings. Advocate for older adults to enhance their access to aging programs.

c. Deliver presentations at community organizations, discussing PCA services and other aging programs. Represent PCA at health fairs, community events, and media opportunities.

d. Provide education to PCA employees to foster a better understanding of the needs of elderly individuals in ethnically diverse communities.

e. Chair PCA's Latino Advisory Council, organizing quarterly meetings and facilitating the Latino Conference Planning Committee.

f. Assist in the planning of meetings, conferences, and events that enhance communication and bridge the gap between the Latino community and aging services.

g. Keep PCA and other agencies informed about relevant materials suitable for ethnically diverse communities.

h. Maintain accurate records and prepare necessary reports for PCA and other stakeholders.

i. Perform other duties as assigned by the supervisor.

Performance Expectations:
  • Network with community partners to increase the flow of referrals to PCA, aging network agencies, and healthcare providers.
  • Enhance awareness of aging services within ethnically diverse communities.
  • Foster sensitivity within PCA and the aging network towards the needs of elderly individuals from diverse backgrounds.
  • Improve the quality and quantity of services provided to target populations.
