Compensation: $33.83 per hour

PATH is looking for passionate individuals eager to assist others in making significant changes in their lives as part of our Interim Housing Team.

ROLE DESCRIPTION
As a member of the Interim Housing Team, the Case Manager plays a crucial role in helping clients escape the cycle of homelessness through a comprehensive and supportive approach.

Key Responsibilities:

  • Conduct thorough intake assessments and collaborate with clients to create Individualized Service Plans (ISPs) that focus on housing objectives, income enhancement, and other personal aspirations, including health, mental wellness, financial stability, vocational pursuits, and social support.
  • Provide essential referrals, connections, and resources to assist clients in achieving their ISP goals.
  • Complete necessary program assessments, such as the VI-SPDAT.
  • Evaluate client strengths and challenges in relation to their short-term and long-term goals, conducting a 90-day review post-initial intake.
  • Maintain accurate documentation as per program and PATH standards, ensuring timely completion of case notes and keeping client files up-to-date and comprehensive.

QUALIFICATIONS:

PREFERRED EXPERIENCE:

  • A minimum of two years in healthcare, case management, homeless services, or a related field.

MINIMUM REQUIREMENTS:

  • All levels of education and experience are welcome.

MANDATORY CONDITIONS:
All candidates must:

  • Provide employment eligibility verification.
  • Obtain CPR/First Aid certification.
  • Successfully complete a Tuberculosis Test, Background Screening, and Drug Test as a condition of employment.

DRIVING REQUIREMENTS:
Driving is a critical function of this role. Candidates must:

  • Have reliable transportation.
  • Possess a valid driver's license.
  • Provide proof of insurance and ownership for personal vehicles used for work purposes.
  • Meet the qualifications for PATH's insurance coverage.
