Position Summary:

The Housing Support Specialist plays a crucial role in assisting families facing homelessness by facilitating their access to permanent housing and essential support services aimed at achieving lasting housing stability. This role involves providing case management services to address various complex needs and minimize obstacles, including referrals to community resources and connections to additional supportive services such as childcare, employment assistance, health and wellness programs, life skills training, and aftercare services to ensure stability.

Key Responsibilities:

  • Conduct intake assessments for all participants to determine the most suitable housing intervention.
  • Assist participants in securing housing, basic necessities, medical and mental health services, substance abuse treatment, and public benefits as applicable; collaborate on developing a Housing Stability or Individual Service Plan.
  • Deliver strengths-based case management and service coordination to help participants obtain and maintain stable housing.
  • Meet with participants weekly (or more frequently as needed) to track progress and address barriers in achieving their Housing Stability or Individual Service Plans.
  • Implement evidence-based practices in service delivery, including Trauma-Informed Care, Intensive Case Management, Motivational Interviewing, Harm Reduction, and Housing First methodologies.
  • Provide crisis intervention services as necessary.
  • Maintain accurate and timely documentation of activities, case management notes, and services rendered to each client using applicable data management systems.
  • Build and sustain relationships with private, public, and nonprofit landlords, including municipal housing departments, to access housing opportunities.
  • Conduct site visits of potential housing units to ensure compliance with habitability standards.
  • Ensure subsidized units align with Fair Market Rate (FMR) guidelines; negotiate rental terms when necessary.
  • Advocate for participants to resolve issues and barriers with landlords when applicable.
  • Foster and maintain relationships with participants, colleagues, and local service providers.
  • Participate in staff and community meetings as required; represent UBH at community events.
  • Prepare and present assigned cases during weekly/monthly Participant Progress Review meetings.
  • Provide regular written updates to the Program Manager and Management as assigned.
  • Perform other duties as required.

Qualifications:

  • Bachelor's Degree in Human Services (e.g., psychology, social work, sociology, or related field) from an accredited institution is required.
  • A minimum of two years of experience providing services to individuals experiencing homelessness, preferably families with children.
  • Exhibit a high level of professionalism.
  • Demonstrated experience working with individuals facing behavioral health challenges, including mental health, substance use, and special education needs.
  • Experience working with diverse populations, with an understanding of cultural, religious, sexual orientation, racial, age, and physical and mental health differences.
  • Familiarity with best practice approaches such as Housing First, Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
  • Knowledge of community resources.
  • Strong written and verbal communication skills.
  • Ability to maintain personal and professional boundaries.
  • Highly motivated self-starter capable of thriving in a high-pressure environment.
  • Creative problem-solving skills.
  • Ability to respond effectively in emergency and crisis situations.
  • Exceptional organizational skills.
  • Bilingual (English/Spanish) preferred.
  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook).
  • Experience using data management systems for client information tracking.
