Reports to: Program Manager
Program: Integrated Care
Department: Mental Health
Work Environment: Combination of field and office settings
Schedule: Standard business hours, Monday to Friday
Status: Full Time/non-management/non-exempt

Key Responsibilities:
- Facilitate access to stable housing and supportive services tailored to individual client needs.
- Conduct thorough assessments and develop personalized intervention plans to address barriers to housing.
- Collaborate with housing authorities and property managers to coordinate client move-ins.
- Promote a strengths-based approach to empower clients in achieving their housing goals.
- Identify and connect clients with suitable permanent housing options, including subsidized and market-rate housing.
- Assist clients in navigating the rental market and completing necessary housing applications.
- Advocate on behalf of clients with landlords and housing authorities to secure housing opportunities.
- Maintain accurate documentation of services provided in compliance with agency standards.
- Engage with community partners, realtors, and service providers to enhance housing access for clients.
- Participate actively in team meetings and professional development opportunities.
Qualifications:
- Bachelor's degree in a relevant field or equivalent experience.
- Proven experience working with homeless populations or individuals facing housing instability.
- Strong organizational, communication, and interpersonal skills.
- Valid driver's license with a clean driving record.
- Ability to work effectively with diverse populations in a culturally sensitive manner.
- Self-motivated and capable of thriving in a dynamic work environment.
- Familiarity with public housing programs and subsidies is preferred.
