We are currently seeking a dedicated member to join our Care Coordination team. The Patient Navigator plays a crucial role in assisting patients enrolled in the Illinois Breast and Cervical Cancer Program (IBCCP). This position focuses on guiding high-risk or newly diagnosed patients through their healthcare journey and organizing support initiatives for cancer survivors and those undergoing treatment.

Key Responsibilities
  • Facilitate patient enrollment in IBCCP, manage billing, budgeting, and caseload documentation.
  • Provide comprehensive case management for patients diagnosed with cancer, collaborating with nursing staff for effective oversight.
  • Offer navigation support for high-risk and newly diagnosed cancer patients, primarily focusing on breast cancer, while also addressing colorectal and cervical cancers.
  • Assist patients in comprehending their diagnosis, treatment alternatives, and available resources, maintaining an up-to-date knowledge of community resources.
  • Coordinate various support groups and events, including monthly support gatherings for breast cancer patients and annual initiatives for cervical and breast cancer awareness.
  • Perform additional duties as assigned.
Qualifications

Education
  • High School diploma or equivalent is required.
  • Preferred: Associate's degree in Community Health, Health Education, LPN, MA, or equivalent.
  • Bilingual proficiency in English and Spanish is required.
Skills and Experience

Required:
  • A minimum of 2 years of experience in a healthcare environment.
  • Proficient in Microsoft Word, Excel, and Outlook.

Preferred:
  • Experience working with specialized populations, particularly in cancer care, or a strong willingness to learn about this demographic.
