Job Overview:

The Patient Care Management Coordinator embodies NM's mission, vision, and values, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all pertinent protocols, procedures, guidelines, and regulatory standards.

As a Registered Nurse, the Patient Care Management Coordinator is tasked with assessing, planning, coordinating, implementing, and evaluating tailored care plans for assigned patients, in addition to providing direct patient care. This role is pivotal in achieving quality outcomes and demonstrating outstanding clinical proficiency. Specific responsibilities may vary depending on the assigned care environment.

Key Responsibilities:

  • Plan, coordinate, direct, and deliver suitable clinical care to patients within the designated care unit
  • Collaborate with the interprofessional team to facilitate ongoing care, focusing on cost-effective strategies to enhance well-being and improve patients' lives
  • Develop, coordinate, and provide individualized education to patients, families, caregivers, and staff, while also serving as a valuable educational resource
  • Act as a communication link between patients, families, caregivers, and the interprofessional team
  • Assist in organizing comprehensive patient care, including necessary follow-up appointments, referrals, tests, treatments, and long-term monitoring
  • Engage in departmental, hospital, or other professional committees, quality improvement initiatives, or process enhancement projects
  • Ensure clinical practices and patient care are in accordance with established policies and procedures
  • Perform additional duties as assigned

Qualifications:

  • Bachelor of Science in Nursing (BSN)
  • Registered Nurse (RN)
  • Basic Life Support (BLS) certification
  • Minimum of 2-5 years of recent and relevant experience

Preferred Qualifications:

  • Master of Science in Nursing (MSN)
  • Specialty certification in the relevant clinical area
