Key Responsibilities Include:

  • Manage home health assessments and system updates
  • Perform outreach and follow-up attempts to patients
  • Build relationships with patients to encourage compliance with care plans
  • Inform and educate patients on their program
  • Complete records in system by performing data entry
  • Encourage patient usage of our programs
  • Conduct check-ins with patients to review care plan goals

Requirements:

  • Bachelor of Social Work, Psychology, LVN, LPN, or RN
  • Knowledge of medical terminology
  • 3 years of care coordination experience
  • PC proficiency including Microsoft Office applications
  • Customer service skills
  • Verbal and written communication skills
  • Current state driver's license and transportation
