Mental Health Crisis Resolution Coordinator

Job Summary: We are seeking a skilled Crisis Case Manager to join our team at Edward-Elmhurst Health. As a Crisis Case Manager, you will work closely with our multidisciplinary team to provide crisis evaluation, service linkage, and post-discharge support to patients in our Emergency Department.
Key Responsibilities:
- Collaborate with medical and psychiatric teams to provide optimal patient care and develop effective crisis intervention plans.
- Coordinate care management with various agencies, including Adult Protective Services, DCFS, SASS, and insurance companies, to ensure seamless patient care.
- Develop and maintain relationships with community resources and service providers to facilitate patient referrals and support.
- Conduct thorough assessments and evaluations to identify patient needs and develop individualized care plans.
- Provide education and support to patients and their families on mental health resources and services.
Requirements:
- Master's Degree or higher in a behavioral health field.
- Minimum of two years of experience in crisis/emergency services, behavioral health setting.
- Knowledge of DSM5 diagnostic criteria, ICD10, and interqual/MCG standards.
- Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Clinical Professional Counselor (LCPC) licensure.
- CPI certification within 90 days of start date.
- Current CPR Certification issued by American Heart Association or Red Cross within 90 days of start date.
