The Behavioral Health Crisis Case Manager plays a critical role in the Emergency Department as part of a multi-disciplinary team, providing crisis evaluation, service linkage, and post-discharge support to patients. This position requires collaboration with medical and psychiatric teams to ensure optimal patient care.
Key Responsibilities- Coordinate care management with various agencies, including Adult Protective Services, DCFS, SASS, psychiatric units, and insurance companies.
- Develop and implement individualized care plans to meet patients' needs.
- Provide education and support to patients and their families on mental health resources and services.
- Collaborate with community-based organizations to ensure seamless transitions of care.
- Participate in quality improvement initiatives to enhance patient outcomes.
- Master's Degree or higher in a behavioral health field.
- High level of communication skills to interact effectively with patients, families, and healthcare teams.
- Knowledge of DSM5 diagnostic criteria, ICD10, and interqual/MCG standards.
- Minimum of two years of previous assessment experience.
- Minimum of two years of experience in crisis/emergency services, behavioral health setting.
- Knowledge of patient rights and law pertaining to mental health.
- Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Clinical Professional Counselor (LCPC) licensure.
- CPI certification within 90 days of start date.
- Current CPR Certification issued by American Heart Association or Red Cross within 90 days of start date.
