Home Care Nurse Client Manager

Positions Available: 2

Location: Candidates must reside in Pinellas County (Largo area) or Pasco County, FL (Holiday/Tarpon Springs).

This role is ideal for a Licensed Practical Nurse (LPN) or Registered Nurse (RN) who will oversee the quality of care provided by nursing staff. This position is not directly involved in hands-on patient care; instead, the focus is on assessing patient requirements, recommending enhancements for care, and ensuring that assigned caregivers deliver outstanding service, among other responsibilities. The role involves both in-office work and home visits, with one position based in Largo and the other in Holiday. This is a full-time, 40-hour workweek position, typically involving two home visits daily. Bring your nursing skills to a compassionate, family-owned organization.

Job Overview

The Home Care Nurse Client Manager is responsible for assessing both medical and non-medical needs and coordinating suitable services to ensure clients receive personalized care tailored to their specific circumstances. Utilizing clinical knowledge, the Nurse Client Manager provides reassurance to clients and their families while advising on critical aspects of senior care. Responsibilities include conducting wellness assessments, managing nursing cases, advocating for clients, and supervising caregivers to maintain high standards of care.

Key Responsibilities:

  • Conduct initial assessments for new clients.
  • Oversee care provision, follow-up, and documentation of all activities.
  • Support the Director of Nursing in training Home Health Aides.
  • Organize medication setups in clients' homes for self-administration.
  • Conduct supervisory visits as per protocols to ensure quality care and review/update care plans.
  • Ensure patient safety and confidentiality at all times.
  • Assist in maintaining accurate client records, ensuring correct dates and signatures, and timely submissions.
