The role involves both in-office work and home visits, with a standard commitment of 40 hours per week.

Job Overview

The Home Care Nurse Client Manager is dedicated to assessing both medical and non-medical requirements and coordinating the necessary services to ensure clients receive the most appropriate care tailored to their individual circumstances. Utilizing clinical knowledge, the Nurse Client Manager offers reassurance to clients and their families while addressing critical aspects of senior care. Responsibilities include conducting wellness evaluations, managing nursing cases, advocating for clients, and supervising caregivers to guarantee high-quality care.

Key Responsibilities:

  • Conduct initial assessments for new clients.
  • Oversee the provision and management of care, ensuring thorough documentation of all activities.
  • Support the Director of Nursing in training Home Health Aides.
  • Organize medication setups in clients' homes for self-administration.
  • Perform supervisory visits as per established protocols to ensure quality care and update care plans as necessary.
  • Ensure patient safety and maintain confidentiality.
  • Assist in the upkeep of client records, ensuring accuracy and timely submission.

Qualifications:

Must possess a valid RN or LPN license.
