This role is ideal for a Licensed Practical Nurse (LPN) or Registered Nurse (RN) who will oversee the quality of care provided by nursing staff. This position focuses on assessing patient requirements, recommending enhancements to care, and ensuring that assigned caregivers deliver outstanding patient services. The role includes both in-office responsibilities and home visits, with a standard workweek of 40 hours.
Job Overview
The Home Care Nurse Client Manager is dedicated to assessing both medical and non-medical needs and coordinating suitable services to ensure clients receive tailored care. Utilizing clinical knowledge, the Nurse Client Manager offers reassurance to clients and their families by advising on essential aspects of senior care. Responsibilities include wellness assessments, nursing case management, and client advocacy, while supervising caregivers to guarantee high-quality service.
Key Responsibilities:
- Conduct initial assessments for new clients.
- Oversee care provision, follow-up, and documentation of all activities.
- Support the Director of Nursing in training Home Health Aides.
- Organize medication setups in clients' homes for self-administration.
- Conduct supervisory visits to ensure adherence to care protocols and update care plans as necessary.
- Ensure patient safety and confidentiality at all times.
- Maintain accurate client records with proper documentation and timely submissions.
Qualifications:
RN or LPN license is required.
