This role is ideal for a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to oversee the quality of care delivered by nursing staff. The position focuses on assessing patient requirements, recommending enhancements to care, and ensuring that assigned caregivers deliver outstanding patient support. When not conducting home visits, the selected candidates will work from the office in Largo or Holiday. This is a full-time, 40-hour workweek position, averaging two home visits daily. Job Overview

The Home Care Nurse Client Manager is responsible for assessing both medical and non-medical needs and coordinating the necessary services to ensure clients receive tailored care. Utilizing clinical knowledge, the Nurse Client Manager provides reassurance to clients and their families while advising on essential aspects of senior care. Responsibilities include wellness assessments, nursing case management, and client advocacy, along with supervising caregivers to maintain high standards of care during home visits.

Key Responsibilities:

  • Conduct initial assessments for new clients.
  • Manage ongoing care provision, supervision, and documentation of all activities.
  • Support the Director of Nursing in training Home Health Aides.
  • Organize medication setups in clients' homes for self-administration.
  • Perform supervisory visits as per protocol to ensure quality care and update care plans.
  • Ensure patient safety and confidentiality are upheld.
  • Assist in maintaining accurate client records, ensuring timely submission with correct dates and signatures.

Qualifications:

RN or LPN license is required.
