Overview and Duties
Open Sky Community Services is dedicated to providing exceptional Adult Community Clinical Services, funded by the Department of Mental Health. Our mission is to implement evidence-based practices within a clinically driven framework. We support individuals diagnosed with mental health conditions or dual diagnoses, who live independently in their own residences or in group settings. Our focus is on facilitating transitions to greater independence while enhancing natural support systems through skill development and goal achievement. Our teams are cohesive, supportive, and multidisciplinary, comprising skilled clinicians, substance use counselors, nurses, peer support staff, housing counselors, outreach counselors, and direct care personnel who deliver compassionate and quality care.
Outreach Counselors serve as vital links between the individuals we support, community organizations, and families, coordinating treatment with various entities including employment and educational resources, emergency service providers, and outpatient clinics. They are dedicated to fostering respectful partnerships, establishing appropriate boundaries, and maintaining a supportive role for both individuals and their families. Outreach Counselors empower individuals to transcend perceived limitations by equipping them with essential tools for success.
Key Responsibilities:
- Implement interventions outlined in the Treatment Plan under the guidance of licensed clinical staff.
- Facilitate activities that promote the development of valued community roles and personal relationships.
- Provide direct support and coaching to enhance social engagement, conducting both individual and group activities aimed at increasing readiness for rehabilitation.
- Assist individuals in securing financial support through employment opportunities or entitlement applications.
- Participate in the On-Call rotation.
- High School Diploma, GED, or equivalent is required.
- A valid Driver's License, reliable and insured vehicle, and a satisfactory driving record are necessary.
- Dependability, eagerness to learn, and openness to new experiences are essential.
