Open Sky Community Services offers a vital program funded by the Department of Mental Health, dedicated to providing evidence-based interventions within a clinically driven framework.
We serve individuals diagnosed with mental health conditions or dual diagnoses, supporting them in their own residences, whether that be homes, apartments, or group living settings.
Our mission is to assist these individuals in transitioning to greater independence while enhancing their natural support systems through skill-building and goal achievement.
Our teams are cohesive, supportive, and multidisciplinary, comprising talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff who deliver compassionate and quality care.
Role of Outreach Counselors
Outreach Counselors serve as a crucial link between the individuals we support, community organizations, and families, facilitating treatment coordination that includes employment and educational services, emergency providers, outpatient clinics, and more.
They are dedicated to fostering a respectful partnership, establishing appropriate boundaries, and maintaining a supportive role with both individuals and their families.
Outreach Counselors empower individuals to transcend perceived limitations by equipping them with the necessary tools for success.
Key Responsibilities:
- Deliver interventions as outlined in the Treatment Plan under the supervision of licensed clinical staff.
- Implement activities that promote the development of valued roles and personal relationships within the community.
- Provide direct support and coaching to assist individuals in socializing, conducting both one-on-one and group activities aimed at enhancing readiness for rehabilitation.
- Ensure individuals receive adequate financial support by aiding in job searches or entitlement applications.
- Participate in the On-Call rotation.
Qualifications
High School Diploma, GED, or equivalent is required.
A valid Driver's License, reliable and insured vehicle, and an acceptable driving record are necessary.
Reliability, eagerness to learn, and openness to new opportunities are essential.
