Position Overview and Duties

Open Sky Community Services is dedicated to providing high-quality, evidence-based support for individuals facing mental health challenges or dual diagnoses. Our mission is to assist clients in achieving greater independence while enhancing their natural support systems through skill development and goal attainment. Our multidisciplinary teams consist of compassionate professionals, including clinicians, substance use counselors, nurses, peer staff, housing advisors, outreach counselors, and direct care personnel, all committed to delivering exceptional care.

As a Mental Health Outreach Specialist, you will serve as a vital link between clients, community organizations, and families, ensuring comprehensive treatment coordination. This includes collaboration with employment and educational resources, emergency service providers, and outpatient facilities. You will foster respectful partnerships, establish appropriate boundaries, and maintain a supportive role for both clients and their families. Your role will empower individuals to transcend perceived limitations by equipping them with essential tools for success.

Key Responsibilities:

  • Implement interventions outlined in the Treatment Plan under the guidance of licensed clinical staff.
  • Facilitate activities that promote the development of meaningful roles and relationships within the community.
  • Provide direct support and coaching to enhance socialization skills through both individual and group activities aimed at improving readiness for rehabilitation.
  • Assist clients in securing financial resources by helping them find employment or apply for entitlements.
  • Participate in the On-Call rotation as required.
Qualifications
  • High School Diploma, GED, or equivalent is required.
  • A valid Driver's License, reliable vehicle, and acceptable driving record are mandatory.
  • Dependability, eagerness to learn, and openness to new experiences are essential.
