Position Overview:
Open Sky Community Services is dedicated to providing Adult Community Clinical Services, a program funded by the Department of Mental Health, aimed at delivering evidence-based interventions within a clinically driven framework.
We serve individuals diagnosed with mental health conditions or dual diagnoses, who live independently in their own residences or in supportive group settings.
Our mission is to assist individuals in transitioning to a more autonomous lifestyle while enhancing their natural support systems through skill-building and the achievement of personal objectives.
Our multidisciplinary teams consist of compassionate professionals, including clinicians, substance use counselors, nurses, peer support staff, housing counselors, outreach counselors, and direct care staff, all committed to providing quality care.
Key Responsibilities:
- Serve as a liaison between individuals served, community organizations, and families to coordinate comprehensive treatment, including employment and educational resources, emergency services, and outpatient clinics.
- Foster a respectful partnership with individuals and their families, setting appropriate boundaries while maintaining a supportive role.
- Empower individuals to overcome perceived limitations by equipping them with essential tools for success.
- Deliver interventions as outlined in the Treatment Plan under the supervision of licensed clinical staff.
- Facilitate activities that promote the development of valued community roles and personal relationships.
- Provide direct support and coaching to enhance socialization skills through both individual and group activities.
- Assist individuals in securing financial support by helping them find employment or apply for benefits.
- Participate in the On-Call rotation as required.
Qualifications:
- High School Diploma, GED, or equivalent is required.
- Valid Driver's License, reliable vehicle, and acceptable driving record are necessary.
- Demonstrated reliability, eagerness to learn, and openness to new experiences.
