Community Support Coordinator

We are seeking a highly skilled and compassionate Clinical Outreach Specialist to join our team at Open Sky Community Services. As a key member of our Adult Community Clinical Services team, you will play a vital role in delivering evidence-based interventions and supporting individuals with mental illness or dual diagnosis in their own homes, apartments, or group living environments.
Key Responsibilities- Act as a liaison between individuals served, community agencies, and families to coordinate treatment and services.
- Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
- Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
- Provide side-by-side support and coaching to help individuals socialize and increase their readiness to engage in rehabilitation.
- Ensure individuals have adequate financial support through assisting with finding gainful employment or applying for entitlements.
- Participate in the On-Call rotation.
- High School Diploma, GED or equivalent, required.
- Valid Driver's License, reliable and insured vehicle, and acceptable driving record, required.
- Reliability, willingness to learn, and being open to new opportunities.
