About the Role

We are seeking a highly skilled and experienced Customer Support Manager - Care Coordination to join our team at Choice of NY. As a key member of our organization, you will be responsible for managing a team of Client Care Coordinators, providing case management services to clients with mental health and/or chronic health conditions.

Key Responsibilities
  • Supervise, train, and develop assigned Client Care Coordinators in their ability to address client needs, resolve issues, advocate, and connect clients to needed resources.
  • Lead and coach assigned team to become increasingly independent in managing day-to-day interactions, developing problem-solving, communication, and advocacy skills.
  • Ensure care plans and activities are aligned with client goals and in compliance with Health Home or Grant Funding requirements.
  • Review client case notes, monitor information, and provide timely and constructive feedback to improve quality of care.
  • Conduct supervision meetings with each Client Care Coordinator, reviewing key metrics and providing guidance on areas for improvement.
  • Monitor quality of care through direct calls to clients, field visits, and side-by-side monitoring.
  • Develop productive relationships with external organizations, such as Department of Social Services, Mental Health Providers, and Primary Care Physicians.
  • Embody a fearless pursuit of excellence, achieving needed outcomes for clients and the organization.
Requirements
  • Proven ability to empathize with clients.
  • Tenacity and passion for this work, balancing objectivity and empathy.
  • Minimum 2-3 years of care-related experience in advocacy, mediation, and crisis management.
  • Bachelor's degree in Human Services, Psychology, or 5 years equivalent work experience.
  • 2 years prior supervisory experience highly preferred.
  • Computer literacy required.
  • Bilingual English/Spanish a plus.
