Position Summary:

The role involves overseeing the coordination of services tailored to address challenges related to homelessness, housing instability, and family dynamics. This includes a variety of issues such as health concerns, substance use, domestic challenges, and lack of support.

Key Responsibilities:
  • Coordinate comprehensive assessments for families engaged in the Permanent Supportive Housing program.
  • Collaborate with case managers to develop and implement Individual Service Plans (ISPs).
  • Facilitate referrals to essential services, including:
    • Medical and behavioral health care
    • Substance abuse treatment
    • Parenting education
    • Family therapy and reunification
    • Youth support initiatives
  • Provide clinical insights and monitor case manager interactions with participants.
  • Review and ensure the quality of case documentation and participant plans.
  • Conduct individual meetings with case managers to strategize and evaluate service effectiveness.
  • Ensure compliance with all relevant regulations and standards set forth by the District of Columbia and MBI Health Services, LLC.
  • Perform additional duties as required.
Qualifications:
  • Master's degree in social work, mental health counseling, or a related discipline.
  • Licensure as a clinical social worker or professional counselor is required.
  • Minimum of two years of experience in case management and direct service delivery.
  • In-depth knowledge of social service delivery systems and practices.
  • Strong communication, problem-solving, and organizational skills.
Compensation:

Salary range: $75,000 - $78,500
