Key Responsibilities:
  • Oversee the coordination of services addressing homelessness, housing challenges, and family dynamics, including issues related to health, substance use, and domestic challenges.
  • Conduct thorough bio-psychological assessments for all family members involved in the Permanent Supportive Housing program.
  • Collaborate with Case Managers to engage families in addressing their unique challenges, which includes:
    • Creating and implementing Individual Service Plans (ISP).
    • Facilitating referrals to:
      • Medical and behavioral health services
      • Substance abuse treatment programs
      • Parenting education
      • Family therapy and reunification services
      • Youth support initiatives
    • Provide clinical insights and monitor case managers' interactions with participants.
    • Review case documentation for quality and accuracy, ensuring compliance with clinical standards.
    • Evaluate and approve participant case plans for effectiveness.
    • Meet with case managers to strategize and assess case management effectiveness.
    • Ensure all communications with participants are accurate and complete.
    • Analyze and update Housing Stability Plans quarterly, tracking participant progress towards their goals.
    • Ensure timely completion of quarterly recertifications and maintain accurate records.
    • Participate in case conferences with district staff to discuss cases requiring intensive management.
    • Perform additional duties as assigned in accordance with organizational standards and regulations.
    Qualifications:
    • Master's degree in social work, mental health counseling, or a related discipline.
    • Must hold a valid clinical social work or professional counseling license.
    • Consideration will be given to graduate social workers or licensed counselors in the District of Columbia.
    • A minimum of two years of experience in case management and mental health services is required.
    • Strong understanding of social service delivery systems and practices.
    • Two years of experience providing counseling or case management to individuals or families facing homelessness or similar challenges.
    • Exceptional oral and written communication skills.
    • Strong problem-solving abilities and time management skills.
