Position Summary

The Case Manager plays a crucial role in delivering personalized case management services to individuals in the Youth Transitional Housing Program. The primary objective is to assist clients in overcoming obstacles to secure and sustain permanent housing through a strengths-based, Housing First methodology. This position involves providing on-site case management support.

Key Responsibilities
  • Deliver comprehensive, client-focused case management to participants in the Youth Transitional Living Program.
  • Offer supportive services to individuals facing homelessness.
  • Conduct face-to-face interactions with clients, dedicating approximately 80% of time to direct engagement.
  • Exhibit empathy and adaptability in addressing the unique needs and requests of clients, promoting wellness and independence.
  • Engage clients to evaluate their individual needs, preferences, and strengths.
  • Formulate, execute, and assess personalized housing case plans with clients to achieve goals related to health, financial management, social support, and daily living skills.
  • Provide financial literacy and budgeting assistance to help clients manage housing costs and daily expenses.
  • Establish trusting relationships with clients, ensuring their safety and well-being, and initiate emergency services when necessary.
  • Oversee medication management for assigned clients as required.
  • Maintain accurate documentation in the Homeless Management Information System (HMIS) and complete case notes for all services provided.
  • Participate in Coordinated Entry Assessment by completing necessary SPDAT assessments.
  • Deliver person-centered supportive services through the Youth Transitional Living Program as needed.
  • Contribute to the planning and development of new initiatives related to client services.
  • Assess client safety and well-being, activating emergency services and additional support as necessary.
  • Build and sustain positive relationships with external agencies that serve homeless individuals.
  • Engage actively in case meetings, staff meetings, and supervision sessions.
  • Participate in training and other professional development opportunities as requested.
  • Assist with volunteer coordination as necessary.
  • Perform additional duties as assigned.
  • Develop, implement, and update individual housing and service plans.
  • Support clients in locating and selecting wraparound services.
  • Assist clients in securing adequate housing resources, including tracking applications and lease agreements.
  • Act as a liaison with landlords and community stakeholders.
  • Attend relevant professional training and conferences.
Qualifications
  • Bachelor's degree in psychology, social work, humanities, social science, or a related field, particularly in youth services.
  • At least two years of relevant experience working with youth, especially those facing homelessness or other societal challenges.
  • In lieu of a degree, a candidate may qualify with five years of direct client work experience in social or human services.
  • Ability to submit documentation within a 24-hour timeframe.
  • Experience providing counseling or case management to vulnerable youth populations.
  • Experience working with victims of domestic violence is preferred.
Skills and Knowledge
  • Familiarity with local social service providers and public benefits programs is advantageous.
  • Crisis management experience is a plus.
  • Strong written and verbal communication skills.
  • Exceptional organizational skills with the ability to manage multiple tasks effectively.
  • Proficient in Microsoft Office Suite and Google Suite/Drive.
  • Ability to maintain confidentiality and demonstrate professionalism.
  • Commitment to equity and social justice principles.
  • Ability to develop individualized housing stabilization plans.
  • Experience in the DC Homeless Service arena, including conducting assessments and using HMIS.
Working Conditions

This position involves a mix of office work and travel to meet with clients across the service area, as well as attending meetings with key stakeholders.

Physical Requirements

The role requires the ability to operate a motor vehicle and possess a valid driver's license. Some lifting may be necessary (up to 20-50 lbs).

Background Checks

The selected candidate will undergo a criminal history background check, including fingerprinting and checks against various registries.
