This is an exciting opportunity to make a meaningful difference in the lives of individuals experiencing homelessness and to be part of a dedicated team committed to providing permanent homes and ongoing support.
Key Responsibilities- Case Management: Provide comprehensive case management services to clients, including assessment, goal setting, and service planning to address their social, health, emotional, and economic needs.
- Service Coordination: Coordinate services with community providers, including healthcare, mental health, substance abuse treatment, and housing services, to ensure clients receive the support they need to achieve stability and independence.
- Advocacy: Advocate for clients' rights and interests, and provide support and guidance to help them navigate complex systems and services.
- Community Engagement: Build and maintain relationships with community partners, including property management, healthcare providers, and social service agencies, to ensure a collaborative and supportive environment for clients.
- Documentation and Reporting: Maintain accurate and timely records of client interactions, progress, and outcomes, and provide regular reports to management and stakeholders.
- Education: Master's degree in Social Work, Psychology, or related field required.
- Experience: Two years of experience providing direct case management services to individuals experiencing homelessness or chronic homelessness.
- Skills: Excellent written and oral communication skills, strong interpersonal and organizational skills, ability to work independently and as part of a team, and proficiency in Microsoft Office Suite and case management software.
- Certifications: Valid driver's license and good driving record required.
- Salary: $47,000 - $53,000 per year, depending on experience.
- Benefits: Comprehensive benefits package, including medical, dental, and vision insurance, and paid time off.
