Position Overview:

The Family Support Coordinator plays a crucial role in assisting families facing various challenges by providing comprehensive case management services. This position is essential in ensuring families receive the necessary support to achieve stability and well-being.

Duties and Responsibilities:

  • Deliver case management services to families in need, ensuring they access vital support services to overcome barriers to stability.
  • Assist families in securing and maintaining stable housing while promoting family strength and resilience.
  • Support families in enhancing their income through employment opportunities, job training, and public assistance programs.
  • Ensure children are placed in suitable daycare or educational settings and engage in enriching extracurricular activities.
  • Oversee team operations, providing supervision and guidance to team members to ensure effective service delivery.
  • Conduct initial intakes, welcoming families, reviewing program policies, and completing psychosocial assessments.
  • Regularly meet with families on caseloads to develop and monitor individualized case plans.
  • Collaborate with specialists in Youth, Employment, and Wellness to address specific family needs as part of the case management strategy.
  • Facilitate communication with treatment, substance abuse, and vocational programs by obtaining signed release forms and coordinating care.
  • Maintain accurate and organized case files, including updated notes, incident reports, and assessments, in line with established protocols.
  • Provide administration and oversight to a team of Housing Stability case managers and youth specialists, ensuring daily operations meet family needs and reporting requirements.
  • Conduct thorough quarterly audits of case documentation for team members, offering constructive feedback to ensure compliance and accuracy.
  • Act as the primary contact for case managers in the field, assessing and planning responses to client crises and unusual incidents.
  • Engage in clinical supervision with senior leadership to ensure client goals are met and services are delivered ethically and effectively.

Minimum Qualifications:

  • A Bachelor's degree is required; a Master's degree in social work or a related field is preferred, along with professional human services licensure.
  • At least three years of experience in social work or a related field is necessary.
  • Experience working with families, homelessness, and substance abuse is highly desirable.
  • Knowledge of community resources or the ability to acquire such knowledge is essential.
  • A commitment to ongoing professional development in relevant areas is expected.
  • A valid driver's license, reliable transportation, and proof of auto insurance are required.
