About the PASSAGES Program

PASSAGES serves as a transitional housing initiative featuring shared living spaces designed for chronically homeless youth up to 24 years of age. Participants can benefit from the program for a duration of up to six years, depending on their age, and receive intensive case management services to foster stability and self-sufficiency.

Role Summary

Reporting to the Program Manager, the multilingual support coordinator will facilitate care coordination, assist with social service navigation, and provide comprehensive support to clients enrolled in the PASSAGES program.

Required Qualifications

  • Bachelor's Degree in Social Work, Psychology, or a related human services field, or three years of relevant experience working with at-risk youth and vulnerable populations, particularly those experiencing homelessness.
  • Proficiency in both Spanish and English is essential; experience working with Latinx communities is advantageous.
  • Knowledge of social services and support organizations within the region is preferred.
  • Excellent communication and organizational skills are mandatory.
  • This is a full-time, exempt position. Candidates must possess a valid driver's license and reliable transportation, pass necessary background checks, and provide any additional information regarding prior records if required.

Key Responsibilities

  • Conduct all necessary admission processes specific to funders when clients are referred to PASSAGES, including intake and orientation regarding program rules and policies.
  • Utilize a competency-based framework in collaboration with PASSAGES staff to develop, implement, and monitor individualized service plans aimed at helping clients achieve self-sufficiency.
  • Connect clients with appropriate social services and actively assist them in navigating these services, including obtaining vital documentation and accessing food and benefits.
  • Facilitate individual counseling sessions with clients on the caseload.
  • Support the planning and execution of program activities, including life skills workshops and relevant training sessions for clients.
  • Provide crisis intervention and on-site counseling as necessary.
  • Conduct regular inspections of the facility and surrounding areas to ensure safety and compliance with program rules.
  • Assist with office duties, including answering phone calls and managing visitor inquiries.
  • Complete documentation of services provided in client case records within 24-48 hours, maintaining accurate and confidential case files and reports to track client progress.
  • Participate in team meetings, training sessions, and community outreach events as required, ensuring completion of mandatory annual training.
  • Be available for evening and weekend shifts as needed.
  • Perform other duties as assigned by the Program Manager or designated supervisor.
