Key Responsibilities:

  • Deliver comprehensive case management services to assist families facing various challenges in achieving stability and access to essential support services;
  • Aid families in securing and maintaining stable housing while fostering family resilience and well-being;
  • Support families in enhancing their income through employment opportunities, vocational training, and public assistance programs;
  • Ensure children are placed in suitable daycare or educational settings and engage in enriching extracurricular activities;
  • Oversee and manage team operations while providing supervision and mentorship to team members;
  • Conduct initial assessments, welcoming families to the program, explaining policies and procedures, and completing psychosocial evaluations;
  • Regularly meet with families on caseload to formulate case plans and provide necessary follow-up;
  • Collaborate consistently with specialists in Youth, Employment, and Wellness to ensure tailored support is available as part of the family's case management strategy;
  • Obtain signed consent forms and maintain ongoing communication with staff at treatment, substance abuse, and vocational programs to ensure coordinated care;
  • Maintain accurate records, including updated case notes, incident reports, assessments, and other documentation in line with established protocols;
  • Provide administration, supervision, and oversight to a team of Housing Stability case managers and/or youth specialists to ensure effective daily operations and reporting;
  • Conduct thorough quarterly audits of team member charts and provide constructive feedback to ensure compliance with documentation standards;
  • Act as the primary contact for case managers in the field, assessing and strategizing responses to client crises and unusual incidents, reviewing reports for clarity and completeness;
  • Engage actively in clinical supervision with senior leadership to ensure client objectives are met and services are delivered ethically and effectively.
Required Qualifications:
  • A minimum of a Bachelor's degree is required; a Master's degree in social work or a related field is preferred, along with professional human services licensure;
  • At least three years of experience in social work or a related field is essential;
  • Experience working with families, homelessness, and/or substance abuse is highly desirable;
  • Knowledge of community resources or the ability to acquire such knowledge is necessary;
  • Commitment to ongoing professional development in relevant areas;
  • A valid driver's license and reliable transportation are required, along with proof of auto insurance.
