Client Care Specialist

WellStone is a leading behavioral health organization dedicated to providing comprehensive support to individuals across various life stages. Our mission is to serve those in need with compassion and expertise.
We are currently seeking a Client Care Specialist to join our team.
The working hours are from 8am to 5pm, full-time, Monday through Friday.
Key Qualifications:
- A high school diploma or GED is required, along with a minimum of 2 years of relevant experience, particularly with individuals facing psychiatric, substance use, or developmental challenges.
- Preferred educational background includes an Associate's, Bachelor's, or Master's degree in a human services discipline.
- Strong verbal and written communication skills in English are essential; proficiency in Spanish is a plus.
- Proficiency in computer applications, including Microsoft Office and Electronic Health Records (EHR), is necessary.
- A valid driver's license in Alabama, a clean driving record over the past five years, and reliable transportation are required.
- Engage with clients to ensure they attend appointments, complete necessary documentation, and manage related tasks.
- Reach out to clients who have missed appointments to encourage their participation and support their recovery journey.
- Collaborate with clients and their families to facilitate progress towards recovery and resilience objectives.
- Assist clients in accessing essential public services, including financial aid, housing, and emergency support.
- Maintain communication with clients during hospitalization, participate in discharge planning, and coordinate with all relevant providers to develop aftercare strategies.
- Perform additional duties as assigned.
