We are seeking a highly skilled Care Coordinator to join our team at WellStone. As a Care Coordinator, you will play a critical role in supporting our clients in achieving their recovery goals.

Key Responsibilities:

  • Follow up with clients regarding appointments, completion of forms, returning forms or receipts, and other similar activities.
  • Contact consumers who have missed appointments in order to re-engage them and promote recovery efforts.
  • Work collaboratively with client/family, focused on supporting client's movement toward recovery/resiliency goals.
  • Aid in accessing public services, including financial, housing, emergency assistance, food, etc.
  • Maintain contact with clients who are hospitalized, participating in discharge planning and communicating with all involved providers in developing aftercare plans.
  • Other duties as assigned.

Requirements:

  • High School Diploma or Equivalent required; Associate's/Bachelor's/Master's degree in a human service-related field preferred.
  • At least 2 years of experience working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • Excellent oral and written communication skills in the English language; Spanish helpful.
  • Computer literacy including use of Microsoft Office, internet applications, and use of Electronic Health Records (EHR).
  • Valid AL driver's license, good 5-year automotive record, reliable transportation.
