Position Overview

WellStone is a leading provider of behavioral health services in North Alabama, dedicated to supporting individuals across various life stages. We are currently seeking a Care Coordinator to join our team in a full-time capacity.

Key Responsibilities
  • Engage with clients to ensure they attend scheduled appointments and complete necessary documentation.
  • Reconnect with clients who have missed appointments to encourage their recovery journey.
  • Collaborate with clients and their families to support their recovery and resilience goals.
  • Assist clients in accessing essential public services, including financial aid, housing, and emergency assistance.
  • Maintain communication with hospitalized clients, participating in discharge planning and coordinating aftercare services with relevant providers.
  • Perform additional duties as assigned.
Qualifications

The ideal candidate will possess:

  • A high school diploma or GED, with a minimum of two years of relevant experience.
  • Experience working with individuals facing psychiatric, substance use, or developmental challenges.
  • A degree in a human services-related field is preferred.
  • Strong verbal and written communication skills in English; proficiency in Spanish is a plus.
  • Proficiency in Microsoft Office, internet applications, and Electronic Health Records (EHR).
  • A valid driver's license and a reliable vehicle.
