Key Responsibilities- Engage with clients to ensure they attend scheduled appointments and complete necessary documentation.
- Reconnect with clients who have missed appointments to encourage their recovery journey.
- Collaborate with clients and their families to support their recovery and resilience goals.
- Assist clients in accessing essential public services, including financial aid, housing, and emergency assistance.
- Maintain communication with hospitalized clients, participating in discharge planning and coordinating aftercare services with relevant providers.
- Perform additional duties as assigned.
The ideal candidate will possess:
- A high school diploma or GED, with a minimum of two years of relevant experience.
- Experience working with individuals facing psychiatric, substance use, or developmental challenges.
- A degree in a human services-related field is preferred.
- Strong verbal and written communication skills in English; proficiency in Spanish is a plus.
- Proficiency in Microsoft Office, internet applications, and Electronic Health Records (EHR).
- A valid driver's license and a reliable vehicle.
