Job Summary

Cullman Regional Medical Center is seeking a skilled Home Health Nurse Specialist to join our team. As a Home Health Nurse Specialist, you will be responsible for evaluating and admitting patients to Home Care, developing plans of care, and providing care to patients according to their individual needs.

Key Responsibilities

  • Evaluate and admit patients to Home Care, developing plans of care that meet their unique needs
  • Act as a case manager, making necessary adjustments to patients' plans of care in response to symptom changes and changes in care needs
  • Review and plan patient care and discharge during scheduled case conference meetings, documenting accordingly
  • Act as a liaison for patients and caregivers with medical professionals and community healthcare partners, striving to lower the cost of care through efficient management of resources
  • Complete all documentation related to patient contact, assessments, and care plans, maintaining a thorough understanding of current Home Care Regulatory Guidelines
  • Provide on-call coverage for Home Care and Hospice as assigned, demonstrating a commitment to exceptional patient experiences and team behavior
  • Uphold and promote patient safety and quality, ensuring that all care is delivered with the highest level of professionalism and expertise

Requirements

  • Associate's degree in Nursing and current licensure by the State of Alabama required
  • Minimum 2 years of experience in a hospital setting, with a background in Med/Surg, Critical Care, or ER
  • Previous experience in Home Care or Hospice is preferred, but not required
  • Strong critical thinking and organizational skills, with the ability to work independently and make sound decisions
  • Flexibility in scheduling, with the ability to prioritize and organize patient care needs
