Role SummaryWe are seeking a Care Coordinator to join our team. This full-time position operates from 8 AM to 5 PM, Monday through Friday.
Qualifications- High School diploma or GED is required, along with a minimum of 2 years of relevant experience.
- Experience must include working with individuals facing psychiatric challenges, substance use issues, or developmental disabilities.
- A degree in a human services-related field (Associate's, Bachelor's, or Master's) is preferred.
- Strong verbal and written communication skills in English are essential; proficiency in Spanish is a plus.
- Proficient in computer applications, including Microsoft Office and Electronic Health Records (EHR).
- A valid driver's license in Alabama, a clean driving record, and reliable transportation are necessary.
The Care Coordinator will:
- Engage with clients to ensure they attend appointments and complete necessary documentation.
- Reach out to clients who have missed appointments to encourage their participation in recovery efforts.
- Collaborate with clients and their families to support their recovery and resilience goals.
- Assist clients in accessing essential public services, such as financial aid, housing support, and emergency assistance.
- Maintain communication with hospitalized clients, participate in discharge planning, and coordinate aftercare services with involved providers.
