ESSENTIAL QUALIFICATIONS AND EXPERIENCE

  • Bachelor's degree in social work, healthcare, or a related discipline
  • 2-3 years of relevant experience, particularly in social work or case management within healthcare settings
  • Familiarity with healthcare systems and programs designed for veterans and military families
  • Knowledge of disabilities, including Serious Mental Illness (SMI) and substance use disorders
  • Proficient in computer applications, including Microsoft Office and Client Management Systems (CMS)

PREFERRED QUALIFICATIONS AND EXPERIENCE

  • Experience with Housing First initiatives, Veteran's Administration (VA), and/or U.S. Military is advantageous
  • Master's degree in social work is preferred
  • Military background is a plus

POSITION SUMMARY

Insight Global is seeking a dedicated Healthcare Navigator to support a client committed to delivering extensive assistance and services to veterans in need. The organization aims to empower veterans to achieve fulfilling lives by providing support in areas such as housing, healthcare, employment, and other vital services. The Healthcare Navigator will offer navigation assistance in healthcare and related supportive services to homeless and at-risk Priority Veteran clients.

Key Responsibilities:

  • Work collaboratively with the Case Management team to create personalized service plans that aid homeless or at-risk Veterans and their families in overcoming health and disability challenges.
  • Provide assistance to referred Veterans and families with specialized health and/or disability requirements, including mental health and substance use issues.
  • Conduct tailored assessments and develop care plans based on the specific circumstances and long-term housing stability goals of Veterans and their families.
  • Engage in weekly meetings with the Director of VC to discuss cases and offer crisis intervention/support to the Case Management team as necessary.
  • Assist referred Veterans and families in navigating healthcare systems, including gaining access to VA healthcare or community care, facilitating enrollment in the VA Medical Center, and managing necessary documentation for enrollment.
  • Complete SOAR training and achieve SOAR certification to help Veterans and families apply for SSDI/SSI benefits through the Social Security Administration.
  • Support Veterans and families with processes related to VA Disability benefits and discharge status, including appeals and obtaining necessary paperwork.
  • Facilitate access to healthcare appointments by helping Veterans identify their health care needs and collaborating with healthcare teams.
  • Assist Veterans and families in utilizing available services, including preventive healthcare.
  • Help identify and address barriers to adhering to recommended healthcare plans, maintaining housing stability, and enhancing overall functioning.
  • Provide client-centered advocacy and support.
  • Assist Veterans and families in understanding and communicating with healthcare providers to make informed decisions regarding their health care.
  • Deliver wellness education to Veterans and families.
  • Maintain accurate and timely case documentation in accordance with SSVF grant guidelines.
  • Actively engage in professional development training and participate in weekly staff meetings and individual supervision.

WORKING ENVIRONMENT:

  • Services are delivered in various settings, including office environments, community locations, and client residences.
  • Occasional after-hours or weekend work may be required.
  • Travel is a necessary component of the role.

COMPENSATION: $21.15/hr
