The Case Manager will play a crucial role in offering tailored assistance to veterans, guiding them through various services and resources to enhance their overall well-being.

Key Responsibilities:

  • Evaluate clients' needs, strengths, and challenges to collaboratively create effective housing stability plans.
  • Deliver comprehensive case management, including regular home visits, housing counseling, client-centered advocacy, referrals, and crisis intervention to foster housing stability.
  • Manage a designated caseload of veterans and/or veteran families, encompassing traditional, Shallow Subsidy, and HUD-VASH cases.
  • Clearly communicate the Priority Veteran program to potential clients, landlords, and the community, while conducting outreach in assigned regions.
  • Maintain ongoing communication with local homeless coalitions, VA Hospitals, shelters, and other relevant organizations.
  • Respond promptly to all inquiries and accurately assess eligibility based on grant guidelines.
  • Ensure timely and precise maintenance of case documentation.
  • Adhere to grant regulations and program policies diligently.
  • Engage in professional development training actively.
  • Participate in weekly staff meetings and individual supervision sessions.

Working Environment:

  • Services are delivered in office settings, community locations, and client residences.
  • Occasional after-hours or weekend work may be required.
  • Travel is necessary for this role.

Essential Skills and Qualifications:

  • Experience in case management, with a solid understanding of trauma-informed care and person-centered practices.
  • Strong interpersonal and communication abilities.
  • Proficient in computer applications, including the Microsoft Office suite and Client Management Systems.
  • Possession of a valid Driver's License and reliable transportation, with a willingness to travel locally approximately 70% of the time.
  • Able to build rapport with clients experiencing homelessness or at risk of homelessness.
  • Successful completion of background checks, including criminal record, drug screening, and TB testing.

Compensation: $19 to $21 per hour
