The Case Coordinator will offer tailored support to veterans, guiding them through various services and resources to enhance their quality of life.
Key Responsibilities:
- Evaluate clients' needs, strengths, and challenges, collaborating with them to create effective housing stability plans.
- Deliver comprehensive case management, including regular home visits, housing counseling, client-centered advocacy, referrals, and crisis intervention to foster housing stability.
- Manage an assigned caseload of veterans and/or veteran families, encompassing traditional, Shallow Subsidy, and HUD-VASH cases.
- Communicate the Priority Veteran program effectively to potential clients, landlords, and the community, while conducting outreach in designated areas.
- Maintain consistent communication with local homeless coalitions, VA Hospitals, shelters, and other relevant organizations.
- Respond promptly to all inquiries and accurately conduct eligibility assessments in accordance with grant guidelines.
- Ensure timely and accurate maintenance of case documentation.
- Adhere to grant regulations and program policies meticulously.
- Engage in ongoing professional development training.
- Participate actively in weekly staff meetings and individual supervision sessions.
Working Conditions:
- Services are provided in various settings, including office environments, community locations, and client residences.
- Occasional after-hours or weekend work may be required.
- Travel is necessary for this role.
Compensation: $21.15/hr
- Conversion Salary: $40,000 - $44,000/yr

Skills and Qualifications:
- Experience in case management, with knowledge of trauma-informed care and person-first practices.
- Exceptional interpersonal and communication abilities.
- Proficient in computer skills, including Microsoft Office suite and Client Management Systems.
- Possession of a valid Driver's License and reliable transportation.
- Able to build rapport with clients experiencing homelessness or at risk of homelessness.
- Background checks, including criminal record, drug screening, and TB testing, are required.
- A Bachelor's Degree in Social Work or a related field is preferred.
- Familiarity with Housing First principles, the Veteran's Administration (VA), and/or the US Military is advantageous.
