Role Overview:

The Case Manager will offer tailored support to veterans, guiding them through various services and resources to enhance their quality of life. This position demands exceptional interpersonal skills, a profound understanding of veterans' challenges, and the capability to collaborate with multiple agencies and service providers.

Key Responsibilities:

  • Evaluate clients' needs, strengths, and obstacles, and collaborate with them to formulate effective housing stability strategies.
  • Deliver comprehensive case management, including regular home visits, housing counseling, client-centered advocacy, referrals, and crisis intervention to foster housing stability.
  • Manage an assigned caseload of veterans and/or veteran families, encompassing traditional, Shallow Subsidy, and HUD-VASH cases.
  • Effectively communicate the Priority Veteran program to potential clients, landlords, and the community, while conducting outreach in designated areas.
  • Maintain ongoing communication with local homeless coalitions, VA Hospitals, shelters, and other relevant organizations.
  • Respond promptly to all inquiries and accurately assess eligibility in accordance with grant guidelines.
  • Ensure timely and precise maintenance of case documentation.
  • Adhere to grant regulations and program policies diligently.
  • Engage in professional development training actively.
  • Participate in weekly staff meetings and individual supervision sessions.

Working Conditions:

Services are delivered in various settings, including office environments, community locations, and client residences. Some after-hours or weekend work may be required, along with travel.

Compensation:

The hourly wage ranges from $19.25 to $21.15, with exact compensation varying based on skills, experience, and education.

Benefits:

Benefit packages commence on the 31st day of employment and include medical, dental, and vision insurance, as well as options for HSA, FSA, and DCFSA accounts. Employees also gain access to a 401k retirement plan with employer matching, along with paid sick leave and other paid time off as mandated by applicable law.
