Position Overview

The role entails overseeing personalized, outcome-focused nursing care by utilizing the nursing process effectively. This includes delivering care, treatment, and services through efficient coordination and execution of a series of tasks that encompass assessment, care planning, and direct patient care, all in line with established departmental and hospital protocols.

Key Responsibilities

1 - PATIENT ASSESSMENT
Conducts thorough evaluations and identifies the needs of patients and their families to ensure high-quality care delivery.

a - Executes pre-anesthesia evaluations, gathering comprehensive health histories and identifying factors that may affect the upcoming surgical procedure.
1. Collects detailed patient health information.
2. Performs a comprehensive review of systems.
3. Gathers all relevant preoperative test results.

b - Completes nursing assessment documentation in accordance with established guidelines.

c - Evaluates patient comfort levels using approved pain assessment tools appropriate for specific age groups.

d - Determines necessary testing based on clinical indicators and risk factors, including age, pre-existing conditions, and the complexity of the surgical procedure.

2 - CARE PLANNING
Develops, discusses, and communicates a prioritized care plan for each patient, collaborating with patients, families, and significant others to address all identified needs.

a - Creates a written care plan that documents patient needs and outlines interventions according to established protocols, including:
Problem identification
Nursing interventions
Additional healthcare team services
Outcome criteria evaluation

b - Aids patients and families in establishing healthcare objectives that are part of the individualized care plan.

c - Formulates patient and family education and discharge plans, documenting them as per policy.

d - Identifies potential issues that may affect the surgical plan and communicates these effectively.

3 - MEDICATION MANAGEMENT
Exhibits the necessary skills and judgment to implement the medical care plan, nursing interventions, and procedures essential for patient care.

a - Demonstrates understanding of hospital policies regarding medication documentation and the completion of medication reconciliation forms.

b - Displays knowledge of commonly prescribed medications, including their actions, dosages, side effects, and utilizes resources to review unfamiliar medications relevant to surgical procedures.

4 - RECORD KEEPING
Maintains accurate and current documentation of nursing care provided to ensure the integration of information for the healthcare team, thereby assuring quality care.

a - The discharge plan accurately reflects the patient and/or significant other's capability to manage care post-discharge.

b - Referrals are made based on identified needs and communicated to the healthcare team as per policy.

5 - TEAM LEADERSHIP
Directs both licensed and non-licensed personnel in delivering care to assigned patients.

6 - SAFETY CULTURE
Promotes a culture of safety through personal accountability and commitment to maintaining a safe environment.

a - Adheres to approved procedures for verifying physician orders, including verbal readbacks to licensed independent practitioners after receiving telephone orders.

b - Complies with current hand hygiene guidelines as outlined by the CDC, as observed by nursing management and peers.

c - Evaluates and periodically reassesses each patient's fall risk using the approved assessment tool, following policy guidelines.

7 - PROFESSIONAL GROWTH
Takes responsibility for ongoing professional development by integrating evidence-based practices, research, and performance improvement initiatives into nursing practice.

a - Remains current with all hospital and unit-based educational requirements.

b - Incorporates insights gained from professional organizations and literature into practice.

8 - COLLABORATION

QUALIFICATIONS

Education Requirements

Graduate of an accredited School of Nursing.

Experience Requirements

Proven proficiency in acute care nursing, with a strong foundation of knowledge and skills.

Licensure Requirements

RN - Registered Nurse.

Required Skills/Certifications

BLS - Basic Life Support.
