Your Role and Responsibilities
The Advanced Palliative Care Nurse Practitioner is a licensed professional nurse with advanced education and clinical training who offers consultative palliative care services to patients facing serious illnesses. This role involves evaluating and assessing health histories, addressing disease prognostication, assisting with advanced care planning, and collaborating with patients, families, and attending providers to recommend symptom management strategies.
Key Qualifications
- Conduct comprehensive history and physical examinations, assess the patient's medical condition and health history, interpret laboratory and diagnostic test results, and document findings in the patient's medical record.
- Provide palliative consultations for patients with serious illnesses, engage in candid discussions about care goals, assist with advanced care planning, and recommend symptom management strategies to attending providers while coordinating hospice services as needed.
- Review clinical information and coordinate patient care in collaboration with the palliative care interdisciplinary team and other healthcare professionals.
- Evaluate treatment protocols and propose intervention options based on current medical literature.
- Consult with collaborating physicians or organizational leadership when the patient's care plan deviates from standardized practices and protocols.
- Graduation from an accredited School of Nursing and an accredited Nurse Practitioner Program.
- Current nursing licensure in the state and CPR certification as mandated by organizational policy.
- Master's or doctoral degree with at least one (1) year of Nurse Practitioner experience, particularly in geriatrics, oncology, palliative care, and/or hospice.
- Accreditation from an approved certifying body for advanced practice nursing as required by state regulations.
- Preferred certification in a specialist area (e.g., ACHPN, AOCN, AWOCN, etc.). If not ACHPN certified, a plan to obtain certification within 18 months of hire is required.
- Strong observational, verbal, and written communication skills, problem-solving abilities, and mathematical skills; nursing competencies as per checklist.
