Key Responsibilities
We are seeking a skilled and empathetic Intake RN or LPN Baylor to work 24 hours per week while being compensated for 32 hours. This role entails working three days per week, specifically on Friday, Saturday, and Sunday, for 8 hours each day.
Your primary responsibilities will include:
- Evaluating the suitability of referrals to the Agency's Intake Department.
- Ensuring timely processing of referrals for home care services.
- Collaborating with Clinical Managers to coordinate daily workloads and assess patient appropriateness for agency services.
- Building relationships and providing education and support to referral sources.
Qualifications
We are looking for candidates who possess:
- A minimum of 3 years of nursing experience.
- At least 1 year of experience in direct home health care is preferred.
- Experience working with Intake CTAs to manage new referrals.
- Strong clinical assessment skills with the ability to prioritize and triage patients effectively.
- Exceptional communication skills and a collaborative approach to teamwork.
- Excellent organizational and time management abilities.
- A current Massachusetts nursing license in good standing.
- Experience with Medicare/Managed Care.
- Familiarity with medical database programs (e.g., Health Wyse).
