Key Responsibilities

The Advanced Practice Palliative Care Nurse is a licensed professional nurse with advanced education and clinical training who offers consultative palliative care services to patients facing serious illnesses. This role involves evaluating and assessing health histories, addressing disease prognostication, assisting with advanced care planning, and collaborating with patients, families, and attending providers to recommend symptom management strategies.

Qualifications We Seek
  • Conduct comprehensive history and physical examinations, assess the patient's medical condition and health history, interpret laboratory and diagnostic test results, and document findings accurately.
  • Provide palliative consultations for patients with serious illnesses, facilitate open discussions regarding goals of care, assist with advanced care planning, and recommend symptom management strategies to attending providers while coordinating hospice services as needed.
  • Review clinical data, coordinate patient care in collaboration with the palliative care interdisciplinary team and other healthcare professionals.
  • Examine treatment protocols and propose intervention options based on current medical literature.
  • Consult with collaborating physicians or organizational leadership when a patient's care plan deviates from established practices and protocols.
  • Hold a degree from an accredited School of Nursing and an accredited Nurse Practitioner Program.
  • Possess current nursing licensure in the state and CPR certification as per organizational policy.
  • Have a Master's or Doctoral degree with at least one (1) year of Nurse Practitioner experience, particularly in geriatrics, oncology, palliative care, and/or hospice.
  • Achieve accreditation from an approved certifying body for advanced practice nursing as mandated by state regulations.
  • Preferred certification in a specialty area (e.g., ACHPN, AOCN, AWOCN, etc.). If not ACHPN certified, a plan to obtain certification within 18 months of hire will be required.
  • Demonstrate excellent observational, verbal, and written communication skills, problem-solving abilities, and mathematical skills; nursing competencies as per checklist.
