Your Role and Responsibilities

We are seeking a skilled, compassionate, and experienced Baylor Intake RN or LPN to join our team, working 24 hours per week while being compensated for 32 hours. This role entails working three days a week, specifically Friday, Saturday, and Sunday, for eight hours each day.

In this position, you will oversee the appropriateness of referrals to the Agency's Intake Department, ensuring timely processing of home care service referrals. You will collaborate with Clinical Managers to coordinate the Agency's daily operations and assess patient suitability for agency services. Building relationships, educating, and supporting referral sources will be key components of your role.

Qualifications We Seek
  • A minimum of 3 years of nursing experience.
  • At least 1 year of direct home health care experience is preferred.
  • Experience working with Intake CTAs to manage new referrals.
  • Strong clinical assessment abilities with prioritization and triage skills.
  • Excellent communication skills and the capacity to function effectively within a team.
  • Outstanding organizational and time management skills.
  • Ability to build relationships, educate, and support referral sources.
  • Current Massachusetts nursing license in good standing.
  • Experience with Medicare/Managed Care is required.
  • Familiarity with medical database programs (e.g., Health Wyse).
