Key Responsibilities

We are seeking a skilled, dedicated, and empathetic Intake RN or LPN Baylor to work 24 hours per week while receiving compensation for 32 hours. This role entails working three days a week, specifically on Friday, Saturday, and Sunday, for 8 hours each day.

Your primary responsibilities will include:
  • Evaluating the suitability of referrals to the Agency's Intake Department.
  • Ensuring timely processing of referrals for home care services.
  • Collaborating with Clinical Managers to coordinate daily workloads and assess patient appropriateness for agency services.
  • Building relationships and providing education and support to referral sources.

Qualifications

We are looking for candidates who possess:
  • A minimum of 3 years of nursing experience.
  • At least 1 year of experience in direct home health care is preferred.
  • Experience working with Intake CTAs to manage new referrals.
  • Strong clinical assessment capabilities and the ability to prioritize and triage patients effectively.
  • Exceptional communication skills and the ability to function within a team environment.
  • Excellent organizational and time management skills.
  • A current Massachusetts nursing license in good standing.
  • Experience with Medicare/Managed Care.
  • Familiarity with medical database programs (e.g., Health Wyse).
