Position Overview

We are seeking a skilled, dedicated, and empathetic Intake RN or LPN Baylor to fulfill a part-time role of 24 hours per week while receiving compensation for 32 hours. This position entails working three days a week, specifically on Friday, Saturday, and Sunday, for 8 hours each day.

Your primary responsibilities will include evaluating the suitability of referrals to the Agency's Intake Department and ensuring timely processing of home care service referrals. You will collaborate with Clinical Managers to help coordinate the Agency's daily operations and assess patient eligibility for services. Building relationships, educating, and supporting referral sources will be key components of your role.

Qualifications

  • A minimum of 3 years of nursing experience.
  • At least 1 year of experience in direct home health care is preferred.
  • Collaboration with Intake CTAs to manage new referrals.
  • Strong clinical assessment capabilities with the ability to prioritize and triage patients effectively.
  • Exceptional communication skills and the ability to function within a team environment.
  • Excellent organizational and time management skills.
  • Ability to build relationships, educate, and support referral sources.
  • Current Massachusetts nursing license in good standing.
  • Experience with Medicare/Managed Care is required.
  • Familiarity with medical database programs (e.g., Health Wyse).
