Position Overview:

The Community Support Manager is tasked with delivering essential social work services to ensure that each Resident achieves or maintains optimal levels of physical, mental, and emotional well-being. This role involves assessing and addressing emotional and behavioral challenges associated with patient health conditions. The Community Support Manager collaborates as part of an interdisciplinary team and may contribute to treatment planning for patients.

Key Responsibilities:

  • Oversee the operations of the Community Support department while adhering to budgetary constraints.
  • Conduct annual performance evaluations for all team members; provide mentorship and training related to their performance. Offer counseling and disciplinary measures as necessary.
  • Facilitate the training and development of staff within the Community Support department.
  • Evaluate each Resident's psychosocial requirements and establish objectives for delivering necessary services, participating in the admissions process as required.
  • Integrate Community Support objectives into the Resident's Care Plan and attend care planning meetings.
  • Assist Residents in acclimating to the facility and foster a supportive environment that encourages ongoing relationships with family and the community.
  • Guide Residents and their families in accessing community resources when services are not directly provided by the facility.
  • Maintain confidential records and conduct interviews with Residents and families as appropriate.
  • Support the development, supervision, and education of staff members.
  • Act as the team lead or designate a team lead for discharge planning within the department.
  • Ensure the completion of necessary components of legal documentation related to guardianship or power of attorney.
  • Coordinate services with mental health providers.
  • Manage services in accordance with regulatory requirements, including overseeing the proper execution and management of relevant programs.
  • Assist the Clinical Interdisciplinary Team in managing resident accommodations.
  • Help Residents and families resolve grievances as assigned.
  • Participate in Clinical Interdisciplinary Team meetings and advocate for Resident Rights.
  • Report any hazardous conditions, equipment malfunctions, and supply shortages to the appropriate personnel.
  • Ensure compliance with established infection control and safety protocols at all times.
  • Uphold the comfort, privacy, and dignity of Residents, interacting with them in a manner that conveys warmth, respect, and promotes a caring atmosphere.
  • Communicate effectively and tactfully with Residents, visitors, families, colleagues, and supervisors.
  • Respond promptly and courteously to call lights when present in Resident care areas.
  • Relay all Resident concerns to the relevant department head.
  • Attend and engage in departmental meetings and training sessions as directed. Participate in ongoing education required for maintaining professional certification or licensure.
  • Understand and adhere to Infection Control guidelines, including hand hygiene practices.
  • Maintain a high level of confidentiality in accordance with privacy regulations, sharing information only on a need-to-know basis.
  • Promote and protect Resident Rights by assisting Residents in making informed choices, treating them with dignity and respect, safeguarding their personal belongings, reporting any suspected abuse or neglect, and supporting independent expression and decision-making in line with applicable laws and regulations.
  • Perform additional duties as assigned.

Education, Training, and Experience:

  • A bachelor's degree in social work is required, along with a minimum of one year of experience in a social service program for the elderly or a related field.
  • Must hold a valid license (full or limited) as a bachelor's level social worker in the State of Michigan.
  • Specific Skills, Knowledge, and Abilities:
  • Knowledge of assessment techniques for evaluating the social and economic aspects of resident care and identifying changes in mood and behavior.
  • Ability to provide continuity and coordination in Resident care, including interviewing Residents and their families and liaising with community resources.
  • Familiarity with community agencies and resources for making referrals related to family and Resident issues.
  • Basic proficiency in business computer applications is required.
  • Effective communication skills with staff, Residents, and families are essential. Must be capable of planning and implementing tasks with minimal supervision.
  • Ability to write clearly and concisely for the preparation of social histories and care plans.
  • Strong listening skills and a compassionate approach to addressing the concerns of Residents with terminal illnesses and their families.
  • A valid driver's license and reliable transportation are necessary for attending off-site meetings and court hearings.
