Purpose & Impact:

The Child Welfare Coordinator's role is to deliver comprehensive case management services to LSF clients and their families, fostering relationships while embodying the agency's core values.

Key Responsibilities:

  • Accept all cases assigned by the unit supervisor and engage with children in their primary residence within two working days of case assignment, ensuring timely completion of Family Assessments.
  • Collaborate with families to create a case plan that reflects their strengths and needs, as well as the circumstances that brought them into care, involving all relevant parties to aim for reunification or other permanency options.
  • Conduct regular visits to the child’s residence, ensuring compliance with required frequency, including unannounced visits at least once per quarter.
  • Document all case activities accurately in the designated database within 48 hours and maintain organized physical case files.
  • Submit service requests and provide timely referrals to community resources, maintaining up-to-date knowledge of available services and eligibility requirements.
  • Prepare and submit court documentation promptly, participating in all necessary court activities.
  • Facilitate individual case staffing and complete all required documentation for staffing and applications.
  • Conduct child safety assessments and develop safety plans as necessary, arranging for emergency services when required.
  • Perform diligent searches for parents and family members as needed, documenting all efforts made.
  • Conduct home studies for prospective placements and provide necessary background information to potential caregivers.
  • Utilize personal transportation to assist children as needed.
  • Ensure completion of all Independent Living functions as required.
  • Attend all relevant appointments, meetings, and training sessions as directed by the supervisor.
  • Serve as the on-call Case Manager as scheduled.
  • Collaborate effectively with Program Directors, fellow Case Managers, placement staff, Protective Investigators, and legal staff.
  • Manage time efficiently to meet all home visit requirements, documentation deadlines, and court obligations.
  • Adhere to Florida Statutes, Administrative Code, and agency policies in managing cases toward established goals.
  • Organize and prioritize work assignments to meet deadlines while maintaining standards of courtesy, communication, teamwork, safety, training, and confidentiality.

Qualifications

Physical Requirements:

  • Valid Florida Driver's License and Insurance.
  • Ability to travel locally and out of the area for professional duties and training.
  • Capability to operate a computer and sit for extended periods while producing written correspondence and progress notes.
  • Flexibility to adapt to irregular hours and perform light lifting as needed.
  • Certification in First Aid/CPR and crisis intervention.

Education/Experience:

  • A minimum of one year of relevant experience is required, with certification to be obtained within one year.
  • A Bachelor's degree in a Human Services field is required, with a preference for Social Work.

Skills/Abilities:

  • Strong written and verbal communication skills.
  • Proficiency in Microsoft Office applications, particularly Word and Excel.
  • Ability to drive locally and throughout the state as required by the position.
  • Understanding of case ownership responsibilities and the integration of services for each child.
  • Demonstrated sensitivity to the cultural and socioeconomic characteristics of the service population.
